Andrea Bertke, MSN, FNP-C
Ms. Bertke graduated from The University of Toledo with a Bachelor of Science in Nursing. She then continued her education at Walden University, where she successfully earned a Master of Science in Nursing and her certification as a Family Nurse Practitioner. Ms. Bertke provides care for patients of all ages, and has been a Certified Nurse Practitioner at Family Health Services since 2015.
Stephanie Breault, FNP-C
Ms. Breault graduated from Northern Michigan University with a Bachelor of Science in Nursing and relocated to Northwest Ohio to be closer to family. She successfully completed her Master of Science in Nursing from Ohio University and obtained her Family Nurse Practitioner certification. She worked at FPG Urgent Care in Clyde, Ohio, for the last six years before joining Family Health Services in March 2023. Stephanie enjoys caring for all ages through all walks of life and spending time with her family and dogs.
Aaron Martinez, DO
Dr. Martinez is a Colorado native who graduated from Metropolitan State University of Denver with a Bachelor of Science in Chemistry. He previously worked in the field of veterinary medicine while completing his Master of Science in Biochemistry. He attended Midwestern University for medical school, and he completed his family medicine residency program in Sandusky at Firelands. Dr. Martinez is happy to call the area his new home and provides primary care to patients of all ages.
Alexa Murray, APRN, FNP-C
Ms. Murray is a Sandusky native who graduated from The Ohio State University with a Bachelor of Science in Neuroscience and soon after became a Registered Nurse, working as a Medical/Surgical RN at the OSU Wexner Medical Center in Columbus. Continuing her education while working at OSU, she went on to receive her Master of Science in Nursing and certification as a Family Nurse Practitioner. She then practiced as an NP with the OSUWMC Department of Neurology before returning home to Sandusky. Alexa is excited to be living near family and on the North Coast again, providing high-quality primary care to patients of all ages with Family Health Services.
Jennifer Richardson, MSN, CNP
Ms. Richardson graduated from Ohio University with her Bachelor of Science in Nursing. She then went on to further her education at University of Cincinnati where she obtained her Family Nurse Practitioner certification and Master of Science in Nursing. Jennifer joins Family Health Services and is able to provide care for patients of all ages.
Laura Spasic, MSN, AGNP-C
Ms. Spasic graduated from The University of Akron with a Bachelor of Science in Nursing. She then went on to successfully receive her Master of Science in Nursing and her certification as an Adult Practitioner from The University of Akron. Laura specializes in adult medicine, and joined Family Health Services in 2017.

Leslie Johnson, MS, RD, LD, Food is Medicine Program Director
Ms. Johnson graduated from Transylvania University in Lexington, KY, with her Bachelor of Arts in Nutrition and Sustainability. She continued her education at the Friedman School of Nutrition Science and Policy at Tufts University, graduating with her Master of Science in Nutrition Communications, Intervention, and Behavior Change. Leslie then went on to complete the requirements to become a Registered Dietitian and completed a year of supervised practice at Duke University Hospital. She is passionate about all people having access to fresh, nutritious food that can help keep their bodies healthy and is excited to provide nutrition counseling for patients at FHS.
Kendall Yuhas, MPH, RD
Ms. Kendall graduated from Kent State University with a Bachelor of Science in Nutrition and then continued her education at Kent State University earning a Master of Public Health with a concentration in social and behavioral sciences. Kendall then went on to complete the requirements to become a Registered Dietitian and completed a yearlong dietetic internship with Sodexo with a focus in weight management. Kendall is happy to call the area her new home and looks forward to helping her patients make sustainable dietary changes that promote overall well-being.

Nicole Beck, BSW, LSW
Ms. Beck graduated with a Bachelor of Science in Social Work from Bowling Green State University and became a Licensed Social Worker in 2021. Nicole has experience in play therapy, addictions, case management, and providing individual counseling for children, teens, couples and adults. Nicole is eager to build relationships within the community and will be providing services in the Norwalk office.
Christy Cox, APRN, PMHNP-BC
Ms. Cox graduated from Owens Community College with an Associates in Registered Nursing Technology. Christy went on to complete a Bachelor of Science in Nursing from the University of Phoenix. She then successfully completed her Post Masters Certificate in Psychiatry from the Northern Kentucky University. Christy provides behavioral health diagnosis, treatment, and medication management, for adults, adolescents, and children, in our Norwalk office.
Sophia Hanson, MSW, LSW
Ms. Hanson graduated from Bowling Green State University with her Bachelor’s in Human Development and Family Studies with a minor in psychology. She continued her education at Bowling Green State University and received her Master’s in Social Work. She is trauma trained in EMDR and TF-CBT. Ms. Hanson provides counseling for children and adolescents at the FHS Sandusky School Clinic.
Melissa Hollada, LPCC
Ms. Hollada graduated with a bachelor’s in science of psychology from University of Phoenix and completed a master’s in clinical counseling from Waldon University in 2016. She has been practicing counseling since as a licensed professional counselor, recently obtaining her LPCC. She is experienced in crisis, addiction, incarceration release back into the community, spirituality, problematic sexual behavior and trauma survivors, as well as EMDR, and personality disorders. Melissa is excited to join the team and work with new patients.
Heather Lambert, MSW, LISW
Heather graduated from Ohio University with a Bachelor of Arts in Social Work and continued her education at Case Western Reserve University earning a Master of Social Work and becoming a Licensed Social Worker. She is trained in EMDR, TF-CBT, and LGBTQ+ youth clinical counseling strategies. Heather provides counseling for children and adolescents at the FHS Edison School Clinic.
Stephanie Lane, MSW, LSW
Ms. Lane is a Florida native who has lived in Ohio for over 20 years. She obtained her Bachelors in Science of Social Work with a minor in Human Development and Family Studies from BGSU and her Masters in Social Work from Case Western Reserve University. In addition, she has received her graduate certificate in Trauma-Informed Care from CWRU, along with a certification in Neurosquential Model of Therapeutics (NMT) and a certificate from Ohio Domestic Violence Network to be qualified to work with individuals who have experienced domestic violence and human trafficking. She sees patients in the office and at one of our local school clinics.
Renee Leber, MSSA, LISW-S, Behavioral Health Executive Director
Ms. Leber graduated from Defiance College and continued her education at Case Western Reserve University for her Master’s in Social Work. She has been licensed since 2007 with a focus in abuse and trauma working with all ages using evidence based practices such as Trauma Focused Cognitive Behavioral Therapy (TF-CBT) with children and Eye Movement Desensitization & Reprocessing (EMDR) with children and adults. Ms. Leber also addresses social and family adjustments and mood disorders such as anxiety, depression, bipolar, and inattention. She is very passionate about empowering those around her to see their own inner strength.
Katelyn Miller, MSW, LISW-S, Lead Therapist for School-Base
Ms. Miller graduated from Bluffton University and continued her education at the University of Tennessee Knoxville where she graduated with a Master of Science in Social Work. She has been a licensed social worker since 2014, obtained her independent licensure in 2018, and her supervisor endorsement in 2019. Katelyn has a variety of experience from mental health counseling, medical social work, hospice/grief, and child welfare. Her training as a therapist includes Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, Mindfulness, Motivational Interviewing, Trauma-Informed, and Strength-Based counseling. She provides counseling for children and adolescents within the FHS Margaretta School Clinic.
Anna Moses, BSW, LSW
Ms. Moses graduated from Bowling Green State University in 2018 with a Bachelor of Science in Social Work and became a Licensed Social Worker. She is providing counseling to children, adolescents, and their families at the Sandusky office and local school clinics.
Kimberly Ollom, MSW, LISW-S
Ms. Ollom graduated with a Bachelor of Arts from Bowling Green State University. She then completed her Master of Social Work from The Ohio State University and is a Licensed Independent Social Worker-Supervisory. She is also grief work and trauma trained in EMDR and TF-CBT, and Certified in DBT. Ms. Ollom provides counseling services for all ages at our Norwalk office.

Ms. Pence graduated from Bowling Green State University in 2023, earning their Bachelor Of Science in Social Work, and becoming a Licensed Social Worker. Lillian is excited to provide counseling for children, teens and families at Western Reserve School District.
Mary Peters, DNP, APRN, PMHNP-BC, FNP-BC, Medical Director
Ms. Peters obtained her Registered Nurse diploma from Providence Hospital, a Bachelor of Science in Nursing from College of Mount Saint Joseph, a Master’s in Nursing and Family Nurse Practitioner degree through the University of Toledo, a Doctor of Nursing through the University of Toledo and Wright State University Consortium, and her Psychiatric Mental Health Nurse Practitioner certification through the University of Cincinnati. Mary looks forward to serving her patients and the community.
Teresa Puckrin, MSW, LSW
Ms. Puckrin graduated from Bowling Green State University in 2019 with a Bachelor of Science in Human Development and Family Studies with a minor in music. She continued her education at The Ohio State University earning her Master of Social Work in 2023 and became a Licensed Social Worker. She provides counseling at the Norwalk office and at the FHS Monroeville City Schools Clinic.
Anthony Schalk, MC, LPC, LCDC III
Mr. Schalk graduated from Kent State University with his bachelor’s degree in criminal justice in 2012 and from Walden University with his Masters in Clinical Counseling in 2021. Anthony is dual licensed having both his LPC, Licensed Practicing Counselor, and his LCDC III, Licensed Chemical Dependency Counselor III. Anthony has over 9 years of experience in community mental health working with children and adults with mental health and substance use concerns. Anthony utilizes Cognitive Behavioral Therapy and Motivational Interviewing in his therapy sessions. He currently provides counseling for youth and adults at the Sandusky location.
Julie Shedron, MSW, LISW-S, Lead Therapist at Norwalk
Ms. Shedron earned a Bachelor’s in Psychology and Sociology from BGSU in 2003. She then earned her Master of Social Work from the University of Illinois at Urbana-Champaign in 2004, with a specialization in community mental health. She is a licensed independent social worker, certified in DBT – dialectical behavior therapy. DBT is a therapy focused on the development of skills to improve how you communicate, manage your emotions & stress and using mindfulness to cope with mental health and addiction symptoms. She is also trauma trained in EMDR. Julie strives to help adolescents and adults improve their self-worth and confidence to problem solve and achieve their life goals.
Cheri Slingwine, MSN, APRN, PMHNP-BC
Ms. Slingwine received her Registered Nurse diploma from Providence Hospital School of Nursing in 1997. She received her Master of Science in Nursing with a specialty in Nursing Education from Walden University. Cheri then continued her education by graduating from Maryville University with her Post Masters Certificate as a Psychiatric Mental Health Nurse Practitioner. Cheri is accepting new patients at our Norwalk office.
Kip Soviak, RN, APRN, FNP-C, PMHNP-BC
Mr. Soviak obtained his RN from Firelands Regional Medical Center School of Nursing. He holds an Associate of Science in Nursing from Bowling Green State University, a Bachelor of Science in Nursing from The Ohio State University, and his Master of Nursing in Family Nurse Practitioner program from Chamberlain University. Kip recently completed his Post Masters Psychiatric Mental Health Nurse Practitioner Certification at Rocky Mountain University of Health Professions. Kip looks forward to working with the community he grew up in.

Mr. Veliz also addresses social and family adjustments and mood disorders such as anxiety, depression, bipolar, and inattention. He has extensive training in working with individuals that have been diagnosed with Autism Spectrum Disorder.

Ms. Wilhelm completed her undergraduate degree from Bowling Green State University and obtained her Master’s of Social Work from The Ohio State University. Kelly has experience in crisis response services, case management, family therapy, and individual counseling to children, teens, and adults. She recently obtained her certification as a certified trauma professional. Her passion is to serve those suffering with mental health issues while reducing barriers that they may face.

Joseph Rizk, DMD – Dental Director
Dr. Joseph Rizk completed his Bachelor of Biological Sciences from Wayne State University and received his Master of Basic Medical Sciences from Wayne State University. Dr. Rizk went on to complete his Doctor of Medicine in Dentistry from Temple Kornberg School of Dentistry. He has over 10 years of experience performing operative, oral surgery, endodontics, removable and fixed dental procedures. He has extensive knowledge and experience of dental clinic operations, maintenance, payroll, budgeting, and recruiting. Dr. Rizk successfully provided complete direction and oversight of the entirety of Mary’s Center Dental Department in Washington D.C. Additionally, Dr. Rizk serves as FHS’ Dental Director.
Fiorella Lamas Castillo, DDS
Dr. Fiorella Lamas Castillo, DDS, received her Bachelor of Stomatology from Cayetano Heredia Peruvian University. Upon completing dental school, she continued her training at the same university in a Master of Stomatology, a diploma in Periodontics, and a Specialty in Periodontics and Implants. She attended the University of Connecticut School of Dental Medicine in the Advanced Education in General Dentistry (AEGD) residency from 2019 to 2021. Dr. Lamas Castillo is a Board-Certified dentist, with experience in restorative, cosmetic, removable & fixed dental procedures, and interests in surgical and regenerative periodontal procedure. Dr. Lamas Castillo speaks fluent English and Spanish and sees patients at our Norwalk location!
Agnish Gholekar, DDS
Dr. Agnish Gholekar, DDS, completed his Bachelor of Dental Surgery from Pravara Institute of Medical Sciences, and received his Advanced Education in General Dentistry (AEGD) from the University of Connecticut School of Dental Medicine. He has experience performing restorative, operative, oral surgery, endodontics, removable & fixed dental procedures. Dr. Gholekar is a Board-Certified dentist and sees patients at our Norwalk location!
Jasmina Saric, DDS
Dr. Jasmina Saric, DDS, graduated from the School of Dental Medicine at the University of Sarajevo, Bosnia and Herzegovina in 2004. She came to the U.S. in December 2009 where she completed her National Board Dental Exam I and II. In 2013, she was accepted to the Advanced Education General Dentistry (AGED) program at NYU Langone in Flint, MI. Upon completing her AEGD, she stayed two additional years at the same FQHC to work as a dental provider. Dr. Saric has experience in restorative dentistry, endodontics, oral surgery, removable and fixed prosthodontics. She enjoys working in community health clinics and making a difference in public health.

David P. Tatro, Sr., MBA, FABC – CEO/CFO
Mr. Tatro comes to Sandusky, Ohio with 30 years’ experience in healthcare management. A native of Western Massachusetts, Mr. Tatro brings a strong background in organizational leadership, strategic planning and revenue enhancement through community partnerships. A former Chief Operations Officer for the last 10 years at Mary’s Center in Washington, D.C., Mr. Tatro grew the company from $8 million in 2009 to over $65 million in 2018. Mr. Tatro was also the founder and Chief Financial Officer for the Capital Clinical Integrated Network in Washington, D.C. which was comprised of multiple FQHC’s and Managed Care Organizations. Mr. Tatro is a graduate of the University of Massachusetts in Amherst, MA and received his MBA from Rivier College in Nashua, NH. He is a Fellow of the Advisory Board Company in Washington, D.C. Additionally, he is an active member of the Capital Chapter of the Nam Knights and a member of Freemasonry, of which both groups support veterans and philanthropically assist local communities. He and his wife Adele will be relocating to the Sandusky area. They have two adult children, Erika Rose and David Jr.
